Indicate Your Interest
By filling out our required interest form you are requesting a consultation with the HealthySteps National Office at ZERO TO THREE. We’ll discuss your interest, your progress securing funding, and any questions you may have.
Explore Fit
Your team will complete our Practice Assessment to ensure that HealthySteps is a good fit. This includes the potential for future success with data collection and reporting. We prioritize sites that align with our strategy to scale HealthySteps in states with current or emerging sustainable funding opportunities for HealthySteps services, and sites that address historical health inequities and serve high numbers or proportions of families with low incomes.
Plan Onboarding
Your practice(s) and the National Office will execute affiliate agreements. We will then work with your team to create an implementation plan. The process of becoming a HealthySteps practice varies but averages 6 months.
Attend Training
The Virtual HealthySteps Institute is an interactive training experience tailored to the needs of your practice(s).
Offer HealthySteps to Your Families
After training, you will now offer HealthySteps to your families!
Receive Support
We know that implementation is an ongoing process, so your team will receive technical assistance for 6-12 months to address any challenges and support practice transformation.
Complete Reporting and Achieve Fidelity
Your practice will complete Annual Site Reporting and receive support to reach model fidelity. After 3 years, your site is expected to achieve fidelity and should see positive outcomes for the children and families you serve, and for your care team.
